We can’t wait to welcome you! Before we dive into your treatment, we’ll need to gather some general information and we request a few quick forms be filled out. Don’t worry, it’s the basics you already know.

Save Time at Your First Visit!

Complete these forms ahead of time to make your first appointment smooth and stress-free. We’ll need your medical history, current medications, consent forms, and a functional/pain assessment.

What Will Happen on Your First Visit

When you arrive, we will need information from you including your insurance card, a picture ID and your copayment or deductible payment.


We will ask you to complete paperwork including medical history, medications, consent forms and functional/pain assessments. 


Your physical therapist will ask questions at your first appointment to learn more about the problem for which you are being seen. You will also be asked questions about how your problem limits your function, and any pain you are having. This helps us quantify the progress you make and provide information on how well we do in helping you recover.


The physical therapist will then do a physical evaluation of your problem and discuss findings, treatment options and your goals. Treatment will begin on your first visit. We will establish a schedule of treatment sessions with you. It is important that if you schedule an appointment you keep it. If you absolutely have to cancel please give 48 hours notice and no less than 24 hours notice so that time slot may be filled by someone else who needs it.


Over the course of your visits, feel free to ask questions and bring up concerns or any problems you are experiencing.
When you are close to completing your treatment, your physical therapist will repeat the functional questions and pain information that was completed on the first visit. You will also receive a patient satisfaction survey which you will return (anonymously if desired) to the front office staff. We read each survey and appreciate any feedback you can provide.


Preparing for your first visit

What to bring:

  • The referral from your physician (if you have one).

  • Your insurance card

  • A picture ID

  • A list of medicines you take (prescriptions and over the counter).

  • Any reports from your doctor you may have about your condition

What to wear:

Please wear comfortable, loose clothing and shoes that will allow you to expose the body area of concern. For example, if you have a shoulder problem, wear or bring a tank top, or for a knee problem, wear shorts. For a back problem, pants and a loose t-shirt should be worn. You may bring these with you and change when you get here (if desired). If you forget, don’t worry, we can provide you with proper attire (style not included)!

When to arrive:

Please arrive 20 minutes prior to your first appointment to complete the required paperwork. We are also happy to mail or email you the evaluation forms before you start.  Please be on time for all your appointments. Your time is set aside for you, as is the person whose appointment follows yours.
