We recognize that understanding health care insurance can be overwhelming, as changes in rules and coverage options happen often. Let our team of insurance and billing specialists help you with all of your questions and concerns.Our front office and billing team work together seamlessly from your very first visit to your very last. We’ll help you to navigate your insurance questions so that you can focus on getting back to your favorite activities and the things you love.

As a courtesy, our patient care coordinators call your insurance company to verify benefits for you, but we encourage you to know your benefits as well: call your insurance member services and ask for outpatient physical therapy benefit packet or description of benefits.

What to Bring on Your First Visit

Please bring the following, if you have them:

  • Insurance card

  • Valid form of Photo ID

  • Any Referrals and/or Prescriptions

  • Medication list

  • Any Xray or MRI reports

Please arrive at least 30 minutes early to complete all required forms.
