Prepare for your first visit to help our physical therapists spend more quality time with you.
We recognize that understanding health care insurance can be overwhelming, as changes in rules and coverage options happen often. Let our team of insurance and billing specialists help you with all of your questions and concerns. Our front office and billing team work together seamlessly from your very first visit to your very last. We’ll help you to navigate your insurance questions so that you can focus on getting back to your favorite activities and the things you love.
As a courtesy, our patient care coordinators call your insurance company to verify benefits for you, but we encourage you to know your benefits as well: call your insurance member services and ask for outpatient physical therapy benefit packet or description of benefits.
If you have questions about billing or insurance information, please contact us at (505)266-3655 between 8:00 am and 5:00 pm Monday-Friday. We look forward to helping you!
What to Bring on Your First Visit
Please bring the following, if you have them:
Insurance card and ID
Referral/Prescription for PT
Any Xray or MRI reports
Please come at least 30 minutes early to complete our forms at the clinic, or fill them out online and either email them back or print and bring them with you (still come at least 10 minutes early so we can start on time!).
***If you fill these out in a browser window (rather than Adobe Acrobat), please press PRINT, and then save as PDF (rather than pressing the download button—that will NOT save your filled in information).***
If you know what body part you are coming in for, or what your doctor is sending you to PT for, please choose a form below and fill it out before coming in. Thank you!
We are "in network" for the following insurance companies:
BCBS of NM
BCBS Medicare Advantage
Healthscope - (Multiplan)
Molina Market Place
Medicare (Part B only)
New Mexico Health Connections
Principal / Admar
Workers Comp - (NM Plans only)
If you don't see your insurance on the list, please contact us to see what we can do for you (things change quickly in the insurance world, and you can always choose to self-pay in order to see one of our physical therapists).
